After submitting, just give us a few days to process them and someone will call you to schedule your first appointment. If you don’t receive a call after 5 business days, please feel free to call and check on it.

If you need to upload any files to us, such as previous lab work, you can use the Upload Files link below. However, you must call and let us know when you upload files since we don’t get an automatic notification.
