We are not currently accepting new primary care patients. However, if you are looking for a thyroid/hormone workup, please feel free to complete the below paperwork and contact us for an appointment.

Prior to scheduling your first appointment, download and complete our new patient paperwork. Please don’t leave anything blank, including signatures. You’ll need to use Adobe Acrobat (not a browser) to apply digital signatures. When you’re ready, you can securely upload it back to us here, then call 850-897-1824 to get scheduled with one of our excellent providers!

We are currently in-network with Medicare, Tricare (but not Prime), and Blue Cross Blue Shield (but not Blue Select). If you’re insured with another company (or have a Blue Select policy) or if you’re uninsured, you’ll need to self-pay for your care at the time of your appointments.

Tricare Prime Patients: We are unable to serve as your Primary Care Physician (PCP) and therefore unable to send referrals for you. You can still be seen at Access Medical Group as a self-pay patient, but referrals to other providers will need to come from your on-base PCP.
